A coroner has found that South Australia's Department for Child Protection missed opportunities that may have prevented the suicide of a 13-year-old boy in state care. Aboriginal and Torres Strait Islander viewers are advised this story contains the image and name of a person who has died which the ABC has been given permission to use by the boy's family.
