Authorities: Colombia soccer plane crash caused by human error

Colombia aviation authorities said the plane which was carrying Brazil's Chapecoense soccer team crashed as a result of errors by the pilot, the airline and Bolivian regulators. The plane, operated by the Bolivian company LaMia, crashed outside of Medellin, killing 71 people. The crash was a result of the pilot not refueling and reporting engine failures in time, officials said. The plane was also overweight by nearly 881 pounds and was not capable of flying at the altitude of its journey, officials concluded.
