Karol Piwowarski's Fatal Crash @ Rajd Wisły 2000

The Lada Samara driven by Jan Koniecki ran over a group of spectators during the 18th special stage of the Rajd Wisły on 30 September 2000. The accident occurred in Zamarski near Cieszyn, in Poland, near the border with the Czech Republic. 15 persons suffered injuries and one of them, 17-year-old Karol Piwowarski, from Częstochowa, died the following day in a hospital in Jastrzębie-Zdrój.

The investigation that followed determined that the brake linings of a rear wheel had been peeled off, this led to a leakage of the hydraulic brake actuation system. It was also found that the car had undergone design changes consisting of the introduction of a single-circuit braking system connecting the pedal and emergency brakes. These changes excluded the possibility of reducing the speed and stopping the car in the event of failure of any component of the braking system.

For these reasons the owner of the car was sentenced to one year and ten months in prison and a fine of 3,000 PLN.
