classified documents by a service member was the result
of an intentional failure to take action by multiple officials.

Based on the preponderance of the evidence
gathered during the investigation, three
individuals in the unit who understood
their duty to report specific information
regarding A1C Teixeira’s intelligence-seeking
and insider threat indicators to security
officials, intentionally failed to do so, Air Force inspector general, via 'The Independent'.
As a result, the Air Force said that action has
been taken against 15 people, including
personnel being relieved of their positions.
