Malfunctioning Ohio state ride throws people into the air, injuring seven and killing one

One person died and seven were injured when the Fire Ball carnival ride malfunctioned at the Ohio State Fair on July 26. Following the accident, Governor John Kasich ordered all rides be shut down.
