MH17 tragedy: 3 years on, questions remain

Monday marks three years since Malaysia Airlines flight MH17 was brought down over Eastern Ukraine.
The tragedy killed all 298 passengers and crew on board.
Since then, blame has frequently been prematurely apportioned - even though the ongoing criminal investigation continues to raise new questions.
